Reactions 1441 - 2 Mar 2013 S Diazepam Various toxicities: case report A 25-year-old man developed various toxicities, including hypotension, hypertension, tachycardia, diplopia, headache, fever, blisters and swelling, following the administration of diazepam. The man was scheduled for herniorraphy, and received IV diazepam 10mg slowly through an infusion line in his right upper limb as premedication for ketamine anaesthesia. He reported severe limb pain as the diazepam was being administered, and within seconds he developed tightness in the chest, double vision, severe headache and dizziness; he became physically aggressive and restless. Examination revealed that he was disoriented in time, person and place. His pulse rate was 120 beats/minute and his BP was 180/110mm Hg. The impression was of an adverse reaction to diazepam. The man received hydrocortisone and promethazine, and his infusion was switched from sodium chloride to glucose. He subsequently calmed down, and his BP decreased to 160/100mm Hg. Herniorraphy was performed using local infiltration with lidocaine and epinephrine [adrenaline]. In the immediate postoperative period, his BP fell to 60/20mm Hg and his pulse rate was 130 beats/minute. He received sodium chloride, plasma expanders, promethazine, hydrocortisone and pentazocine. On postoperative day 1, he developed a high grade fever and his headache continued. His right upper limb was swollen and tender. Blisters developed all over his body. Fluid, steroid and antihistamine were continued. He was treated for malaria, and started amoxicillin/cloxacillin and metronidazole. On day 2, his limb oedema was subsiding, his pulse was 88 beats/minute and his BP was 120/80mm Hg. On day 3, he had improved; his blisters were resolving, and his pulse and BP were normal and stable. He was discharged on day 7; his blisters disappeared. Author comment: "Idiosyncrasies or peculiar mannerism is an abnormal reaction of a person to [an] otherwise normal substance in [an] abnormal manner. This was clearly demonstrated in this case . . .
