Summary and Conclusions
CvLPRIT demonstrated 55% reduction in MACE in those patients presenting for P-PCI when Non-IRA artery is also treated on index admission
With no adverse safety signal
Hard events (death, MI, HF) similarly reduced (5 v 13%) compared to repeat revascularisation (4.7% v 8.2%)
►Indicates in-hospital treatment of the N-IRA seen during P-PCI results in improved clinical outcomes
► Suggests this strategy may be need to be considered by future
STEMI Guideline Committees

• Éviter les perdus de vue pour les patients refusant de revenir
• Éviter une nouvelle ponction
• Raccourcir la durée d’hospitalisation
• Faire profiter précocement les patients de la divergence des courbes de MACE qui est précoce.
• Une lésion serrée sur une artère autre que celle de l’infarctus n’a pas la même gravité que chez un patient stable
• Le bras « culprit only » a bénéficié d’un traitement optimal

Conclusions
• Les arguments pour une revascularisation complète des lésions coupables et non coupables dans le même temps sont solides
• Si ce n’est dans le même temps, il faut au moins la faire dans la même hospitalisation
• Le bon sens clinique, l’expérience de l’opérateur et surtout l’étude des associations lésionnelles doivent permettre d’optimiser la prise en charge
• L’association d’une lésion coupable située sur la CD ou la CX avec une lésion serrée non coupable de l’IVA a probablement une incidence pronostique plus importante que l’inverse…mais aucune étude ne permet actuellement de le montrer. Et donc aucune recommandation ne peut le stipuler !
