TRUVADA® – Abbreviated information for professionalsCOMP: Each fi lm-coated tablet contains 200 mg of emtricitabine and 245 mg of tenofovir diso-proxil. IND: Treatment of antiretroviral naïve HIV-1 infected adults in combination with a non-nucleo-side reverse transcriptase inhibitor or a protease inhibitor. DOS: 1 fi lm-coated tablet each day with a meal. CI: Hypersensitivity to one of the active substances, severe kidney dysfunction, patients requiring dialysis. W&P: Do not administer concomitantly with emtricitabine, tenofovir, cytidine an-alogues, cidofovir, adefovir dipivoxil or didanosine. Not to be taken with liver insuffi ciency, K65R mutation.Care advised with liver disorders, HBV or HCV coinfections and reduced bone mineral density. Monitor liver function. In patients with prior liver dysfunction and signs of deterioration, treatment should be interrupted or discontinued. Regular monitoring of kidney function (serum creat-inine and serum phosphate) is recommended before treatment with Truvada starts. In the fi rst year of treatment, the recommended interval is every four weeks and every three months thereafter. The dosage interval should be modifi ed in patients with CrCl of 30 - 49 ml/min. Treatment to be dis-continued in patients with hyperlactataemia and metabolic acidosis/lactic acidosis, progressive hepatomegaly, rapidly increasing transaminases values. IA: Truvada should be avoided with con-comitant or recent treatment with nephrotoxic medicinal products. Care is advised with concurrent administration of tipranavir, lopinavir/ritonavir, atazanavir, atazanavir/ritonavir, ACE inhibitors and NSAR. PRG/LAC: Only use when it is clearly necessary. Breastfeeding should be avoided. Most common. UE: Hypophosphataemia, headaches, giddiness, diarrhoea, vomiting, nausea, increased creatinine kinase. Dispensing category: A. MAH: Gilead Sciences Switzerland Sàrl, Postal address: Turmstrasse 28, 6300 Zug. Unabbreviated information for professionals under www.swissmedicinfo.ch. CH-GS-201406-201207E
Important information for monitoring the renal function while prescribing Truvada®
It is recommended that creatinine clearance is calculated in all patients prior to initiating therapy. Renal function (creatinine clearance and serum phosphate) should be monitored every 4 weeks during the fi rst year and then every 3 months. If serum phosphate is <1.5 mg/dl (0.48 mmol/l) or creatinine clearance is decreased to <50 ml/min in any patient receiving Truvada®, renal function must be re-evaluated within one week. If creatinine clearance is confi rmed as <50 ml/min or serum phosphate decreases to <1.0 mg/dl (0.32 mmol/l) discontinuation of Truvada® should be considered. Truvada® should only be used in patients with impaired renal function if the potential benefi ts of treatment are considered to outweigh the potential risks. For patients with creatinine clearance <50 ml/min, adjustment in the dosing interval for Truvada® is recommended as per the SmPC. Truvada is contraindicated for patients with severe renal impairment (creatinine clearance <30 ml/min). Use of Truvada® should be avoided with concurrent or recent use of a nephrotoxic medicinal product. If concomitant use of Truvada® and nephrotoxic agents is unavoidable, renal function must be monitored weekly.
SOVALDI® – Abbreviated information for professionalsCOMP: Each fi lm-coated tablet contains 400 mg Sofosbuvir. IND: Sovaldi is indicated in combination with other medicinal products for the treatment of chronic hepatitis C in adults. DOS: The recommended dose is one 400 mg tablet, taken orally, once daily with food. No data available for patients with renal insuffi ciency, hemodialysis, end stage renal disease and decompensated cirrhosis. CI: Hypersensitivity to the active substance or to any of the excipients. W&P: Not recommended for administration as monotherapy. No data available for children and adolescents aged <18 years. IA: No concomitant use with P-gp-inducers (e.g Rifampicin und St.John’s wort) nor with Modafi nil, Rifabutin, Rifapentin, Carbamazepin, Phenytoin, Phenobarbital, Oxcarbazepin, Telaprevir or Boceprevir. PRG/LAC: Must not be taken during pregnancy or when breast-feeding. UE: very common: haemoglobin decrease, anaemia, neutropenia, lymphocytopenia, thrombocytopenia, leukocytopenia, decreased ap-petite, insomnia, headache, dizziness, dyspnoea, cough, diarrhea, nausea, vomiting, blood bilirubin increase, pruritus, rash, arthralgia, myalgia, muscle spasms, infl uenza like illness, pain, fever, fatigue, irritability.
