I. 목적

Inova의 사명은 저희들에게 특별한 봉사의 기회가 주어진 모든 커뮤니티의 모든 사람들에게 - 모든 기회와 모든 만남에서 - 세계적 수준의 보건을 제공하는 것입니다. 이 사명을 달성하기 위하여 세금 면제 보건 조직인 Inova는 사람들에게 그들의 지불 능력과 무관하게 응급 및 의료 필수 보건의료를 제공합니다.

II. 정책

Inova는 자체 자원을 책임 있게 관리하고, Inova가 필요한 최대한의 사람들에게 적절한 수준의 도움을 제공하기 위하여 재정보조 정책 (Financial Assistance Policy (FAP))을 수립했습니다. 이 정책은 또한 지방 정부들과 맺은 모든 구속력 있는 협정에, 그리고 내국세법 501(r) 조를 포함하여 적용되는 모든 주 및 연방 법과 규정에 완벽히 부합하도록 설계했습니다. 이 정책에 의하여, 각각의 Inova의 병원 시설과 기타 포괄대상 기관과 포괄대상 전문인이 시행하는 응급 및 이밖의 의료 필수 서비스를 받는 자격보유자들에게 재정지원이 제공됩니다. 이 정책에 포함되는 내용입니다:

o 미불 상태가 발생할 경우 Inova가 취할 수 있는 조치,

이 정책이 포괄하는 어떠한 Inova 병원, 기관, 또는 전문인도 재정보조 자격이 있는 사람에게

일반적인 청구금액보다 많은 금액을 청구하지 않을 것입니다. 행정처리 이 정책은 시행 시설과 기관에게 적용되는 공적 기관들이 승인했습니다.

III. 정의 이 정책의 취지에 의하여, 아래 용어의 정의는 다음과 같습니다:

• 일반적인 청구금액 (Amounts Generally Billed (AGB)): 501(r)조에 의거, 병원은 진료에 대한 병원의 총 비용을 1 퍼센티지 이상으로 된 총 비용 비율(AGB 퍼센티지)만큼 곱하여 AGB를 결정할 수 있습니다. 병원은 매년 자체 AGB 퍼센티지를 산정해야 하며 이때 응급 및 의료 필수 진료에 대한 전체 청구액 합계를 이 청구에 대한 총 비용 합계로 나누어서 계산합니다. Inova의 AGB 계산 방법에 관한 추가 정보는 이 정책집 10쪽을 참조하십시오. AGB 그리고/또는 계산방법에 관한 추가 정보(무료로 가능함)에 관해서는, Inova의 재정지원과 연락처인 571-423-5880에 접촉하시기 바랍니다.

• 포괄대상 기관: Inova 기관으로서 응급 및 기타 의료 필수 진료를 제공하는 곳이며, 본 재정보조 정책이 적용되는 곳입니다:

o Inova 병원 시설: • Inova Alexandria Hospital • Inova Fair Oaks Hospital • Inova Fairfax Medical Campus • Inova Loudoun Hospital • Inova Mount Vernon Hospital

• 포괄대상 전문인들: Inova 유관 내과의사들과 기타 전문인들로서 이 재정보조 정책이 적용되는 부분 집단. 포괄 대상 기관과 포괄 대상 전문인의 전체 목록은 부록 B를 참조하세요.

• 유자격 서비스: Inova가 포괄하는 기관과 전문인들이 제공하는 서비스로서 재정지원 대상 자격이 있어야 하며, 다음을 포함합니다: (1) 응급실 시설에서 제공되는 응급의료 서비스; (2) 응급실 시설이 아닌 곳에서 이뤄지지지만 생명이 위협받는 환경에 대응하여 제공되는 서비스; 그리고 (3) 기타 의료적 필수적인 서비스.

• 응급 의료 조건: 메디케어 참여 병원은 사회보장법의 1867조 (42 U.S.C. 1395dd)에 성문화 된

응급 의료치료와 노동법 (Emergency Medical Treatment and Labor Act (EMTALA)) 요건을 지켜야 합니다. 환자/보호자의 재정보조 상태, 보험 상태, 지불능력, 현재와 과거의 징수 상태, 또는 여하한 채무불이행 여부와 상관없이 응급 치료를 받기 위해 내원하는 환자가 거부되어서는 안 되며, 본 정책의 어떤 부분도 이러한 치료를 거부하도록 작성되지 않았습니다.

• 가족: 국세청 규칙에 의거, 환자가 누군가를 자신의 소득신고에서 피부양자로 신청한다면, 그들은 재정보조 규정의 취지에 부합하는 가족 구성원으로 간주됩니다. Inova는 결정을 내릴 때 동일한 지침을 따릅니다. 신청서의 지원 명단에 중요한 타인이 있다면 이 사람에 대한 소득증빙을 요청할 것입니다.

• 가족 소득: 가족 소득은 다음 요건에 의하여 결정됩니다: o 세금 납부 이전을 기준으로 결정합니다; o 포함 내용: 수입, 실업 보상금, 노동자 보상금, 사회보장, 생활보조금, 공공부조, 퇴역군인 연금, 사별 가족 수당, 퇴직금 또는 퇴직 소득, 이자, 배당금, 임대료, 로열티, 부동산 소득, 신탁, 교육 보조금, 별거수당, 자녀 양육비, 가구원이 아닌 곳에서 비롯되는 보조금, 그리고 기타 보조적으로 이뤄지는 소득;

o 자본 손실은 제외합니다; 그리고 o IRS 지침에 따라서, 가족 가운데 어떤 사람이 환자에 대한 환급을 신청한다면 그들은 가족 구성원으로 간주되며 신청한 사람의 소득이 포함됩니다.

접수 가능한 문서의 종합적인 목록은 부록 C에 있습니다.

• FAP-유자격자: Inova가 이 재정보조 정책의 취지에 의한 재정보조를 받을 자격이 있다고 결정한 사람.

• 연방 빈곤 기준 (Federal Poverty Guidelines (FPG)): 빈곤 기준은 미국 보건 및 인적 서비스국이 정기적으로 연방 등록소에 업데이트 합니다(부록 A 참조).

• 총비용: 조직이 환자 치료 서비스에 대해 설정한 총 기준 요율에 의한 전체 비용으로서 수입에서 공제금액을 적용하기 이전 금액.

• 소득 문서: 접수 가능한 가족 소득 문서는 다음 가운데 적어도 2가지 종류의 문서가 포함되어야 합니다:

o 가장 최근의 소득신고서 사본; o 가장 최근의 급여 명세서 2건의 사본; o 다른 양식으로 된 제삼자 소득 증명 양식으로서 Inova에 접수 가능한 것으로 간주되는 것은 주택 리스 또는 구입 신청서, 자동차 리스 또는 임차 신청서, 또는 이 밖의 소득 증명이 필요한 신청서와 같은 것입니다.


• 의료 필수: 즉시 치료하지 않으면 해당 환자의 건강 상태가 부작용 변화를 겪게 되는 조건에 대한 서비스입니다. 비만 서비스, 미용 절차, 그리고 특정한 기타 서비스들은 본 재정보조 정책의 대상이 아닙니다.

• Inova서비스 지역: Inova가 서비스하는 지리적 지역 – 본 증서의 취지에 의한 포괄범위는 버지니아 주 지역을 포함합니다.

• 보험 미가입: 환자에게 자신의 지불 의무를 지원하는데 적당한 보험 또는 제삼자 보조가 없는 경우입니다.

• 보험 불충분 가입: 환자에게 일정한 수준의 보험 또는 제삼자 지불이 가능하지만 자부담 금액이 여전히 자신의 지불 능력을 넘는 경우입니다.

IV. 자격 자격 기준과 재정보조 가능성 Inova가 사람들에게 재정보조를 제공하는 것은 당사자의 가구 소득, 가처분 자산, 그리고 정부 제공 또는 정부 보조 건강보험 프로그램 자격을 기준으로 합니다. Inova 병원 시설, 포괄 해당 기관, 포괄 해당 전문인이 제공하는 적격 서비스에 대한 보험 미가입자 또는 보험 불충분 가입자의 재정보조는 아래 기준을 근거로 하여 결정합니다. 1. 응급 의료 조건 또는 비응급 의료 필수서비스에 대한 재정 보조. Inova의 재정보조 프로그램은 응급 및 의료 필수 진료에 대한 지불을 할 수 없는 사람들을 위한 마지막 대책이 되고자 하는 것이 목적입니다. 그러므로 재정보조 신청자는 받게 되는 동일한 의료진료를 감당할 수 있는 여하한 지역, 주, 또는 연방 프로그램에 대한 조사와 신청절차를 지켜야 하며, 여기에는 여행자 건강 프로그램 또는 소속된 사람을 위해 외국 정부 또는 국제 조직/기업이 수행하는 여하한 조직적 프로그램이 포함됩니다.

A. 거주 요건: 응급 의료 조건 또는 비응급 의료 필수서비스에 대한 재정 보조는 Inova의 서비스 지역에서 살고 있으며 적격 서비스가 처음 시행된 날 이전 90일 이상 기간 동안 확정된 거주상태를 유지하는 사람에게 가능합니다. 거주요건은 여성, 자녀, 가족에 대한 Inova 케어 클리닉에는 적용하지 않습니다.

해당하는 사람은 아래 목록 가운데 현재 주소가 포함된 두 가지 자료를 제시하여 Inova서비스 지역 에 거주한다는 사실을 증명해야 합니다:

• 버지니아 운전면허증 또는 주 당국이 발행한 ID 카드로서 • 월별 은행 거래내역서. 은행에서 서비스가 시행 시점에서 90일 이상 이전에

발행한 것이어야 함; • 현재 자동차 또는 생명 보험 청구서; • 미국 국세청 (Internal Revenue Service (IRS)) 세금 보고용 W-2 양식 또는 1099

양식으로서 18개월 이상 되지 않은 것; • 작년 이후의 미국 또는 버지니아 소득세 신고 양식; • 신청자에게 발급된 공과금 청구서. 예를 들자면 가스, 전기, 하수도, 수도, 케이블

또는 전화요금 청구서; • 임대차계약서; • 지역 등록소에서 신청자에게 우편 발송한 버지니아 유권자 등록카드; • 작년 이후에 버지니아 주 또는 버지니아 지역에서 납부한 사람의 재산세 또는

부동산세 영수증; • 버지니아 교육부의 등록 증명서 양식; 그리고/또는 • 미국 주 행정구역 또는 테리토리가 인정하는 학교가 발행하는 학교

기록/성적증명서의 인증받은 양식. • 공증 받은 주거증명서


B. 위에 적시된 거주 요건과 아래의 소득 기준을 만족하는 사람에게는 세 가지 범주의 재정보조가 가능합니다.

1. 보험 미가입 당사자: 가족 소득이 현행 연방 빈곤 기준 (Federal Poverty Guidelines (FPG))의 400% 미만인 보험 미가입 사람은 100% 재정보조 할인(무료 진료)이 가능합니다.

2. 보험 불충분 가입자: 가족 소득이 현행 FPG의 400% 이하인 사람으로서 보험에 가입했으나 부분적으로 포괄되는 사람(예: 보험 불충분 가입자로서 치료를 받은 이후에 지불 가능하지 않은 잔여 책임이 남은 사람), 기타 본 정책에서 기술하는 자격 해당자, 그리고 자신의 보험회사가 Inova에게 보험 지불을 한 이후 잔액에 대하여 재정보조 할인 지불하도록 허용하는 사람에 대해서도 100% 재정보조 (무료 진료)가 가능합니다. Inova가 참여하지 않는 전문인에 의한 보험 플랜을

보유하는 사람에 대한 개인 부담금, 코인슈어런스 그리고/또는 이 밖의 환자 의무금에 대해서는 Inova가 포기할 수 없습니다.

3. 대형 재난 재정보조: 위 기준을 근거로 하는 무료진료 자격이 없으며, 제공된 서비스의 서비스 성격과 범위로 인하여, 가구 소득과 기타 잠재적 가용 자원에 의해서는 진료 관련 재정적 의무가 심각해지는 환자에게는 대형재해 재정보조가 가능합니다. 이러한 환경에 해당하는 환자는 가족 소득 또는 일반적인 청구금액의 30% 미만 까지만 책임을 집니다.

2. 사전 협의 요율. 서비스에 대한 사전협의 할인(패키지 프라이싱)을 받는 환자는 재정보조 자격이 없습니다. 3. 차별금지. 재정보조 지원은 금전적 필요에 대한 개인적 판단을 근거로 하며, 연령, 성별, 성정체성, 인종, 출신국, 사회적 또는 이민 상태, 성적 취향, 또는 소속된 종교에 대해서는 고려하지 않습니다. Inova는 또한 응급 의료 조건에 있는 사람에게 진료를 제공할 때, 그들이 재정보조 자격이 있는지 여부와는 상관없이 수행합니다. 4. 정부 지원 또는 보조 보험 또는 민간 보험에 대한 신청 요건. Inova 재정보조 프로그램은 보험정책이 아닙니다. Inova는 보험 미가입 사람이 정부 지원(예: 메디케이드) 또는 정부 보조 건강보험 자격이 있다면 해당되는 사람에게 이 보험/플랜에 신청하도록 요청하며, 여기에는 여행자 건강 프로그램 또는 여하한 조직적 프로그램으로서 외국 정부 또는 국제 조직/기업이 소속 인원을 위해 운영하는 것이 포함됩니다. 정부 지원 플랜 또는 고용주 기준 플랜 중 어느 것이라도 통하여 보험을 구매할 수 있는 사람은 이렇게 하도록 요청받는데, 이는 그들의 보건 서비스 접근을 확실히 하기 위하는 것이며 그들의 전반적인 사람 건강과 자신의 재산을 보호하기 위하는 것입니다. 5. 추정에 의한 재정보조.

A. Inova는 모든 환자 또는 환자 보호자가 재정보조 신청서를 작성하거나 필요한 서류를 마련할 수 없다는 것을 인지합니다. 이런 경우에, Inova는 제삼자가 환자의 또는 환자 보호자의 정보을 이용하여 재정보조 자격이 되는지 산정하여 환자가 추정에 의한 재정보조 자격이 되는 것으로 간주할 수 있습니다.

B. 계정이 추정에 의한 재정보조 자격이 있는 것으로 간주되면, 해당 환자는 자신의 계정에 대한 의무 금액에 대해 100%의 재정보조 할인을 받습니다.

C. Inova는 어떤 사람이 다음과 같은 조건이거나 자산조사 기준 지급 프로그램에 해당할 경우, 그 사람에게 추정에 의한 재정보조 자격이 있는 것으로 간주할 수 있습니다:

1. 홈리스; 2. 거주장소가 없는 사망자; 3. 식료품비 보조프로그램 (Supplemental Nutrition Assistance Program (SNAP)); 4. 응급 메디케이드 자격이 있는 환자는 메디케이드 프로그램의 포괄 대상이 아닌 응급 또는 의료필수 서비스와 관련된 보조에 대한 자격이 있습니다;

5. 로컬 카운티 빈곤 프로그램 자격이 있는 환자는 해당 프로그램의 포괄 대상은 아닌 응급 또는 의료필수 서비스와 관련된 보조에 대한 자격이 있습니다.

6. 이 증서의 예외 각 Inova 시설의 예산 사이클 부소장, 인구 보건 부소장, 재정 부소장은 이 정책에 의한 재정보조 자격이 되지 않는 사람에 대해 사례별로 판별하여 재정보조 지원을 결정할 권한이 있습니다. 7. 정부가 포괄대상에서 거부할 경우 사람의 적격성. 저소득으로 인하여 정부의 지원 또는 정부의 보조를 받는 건강보험을 신청했으나 결과적으로 거부된 사람으로서 그 이유가 기준 초과 소득 또는 자원 때문이거나, 그리고/또는 이러한 보험 프로그램의 긴급/장애 요건에 적합하지 않기 때문이라면, 이 사람에게는 보조가 가능합니다. 8. 환자의 항변권. 환자가 Inova의 재정보조 자격 결정에 동의하지 않는다면, 이 사람은 결정일로부터 45일 이내에 항변할 수 있습니다. Inova의 재정보조 사무국은 이 항변을 검토하고 60일 이내에 최종 결정을 내리기 위한 의사소통을 합니다.

V. 신청 절차

재정보조를 신청하는 방법 Inova는 작성된 재정보조 신청서(신청서)를 기반으로 재정보조를 지급합니다. Inova의 재정보조 정책에 관한 신청서와 추가 정보는 다음 온라인에서 구할 수 있습니다: http://www.inova.org/patient-and-visitor-information/financial-help/index.jsp. 신청서에는 소득 문서와, 특정 자산에 관한 정보가 필요합니다(퇴직 계정 잔고, 은행계좌 잔고, 신청자가 해당 장소 또는 거주지의 소유 또는 임차 여부). 사람들은 신청서를 작성하여, 작성된 양식을 필요한 문서와 함께 다음 주소로 반송해야 합니다:

재정보조 지원국의 연락처 571-423-5880에 접촉하는 사람들은 신청 절차와 관련된 도움을 받을 수 있습니다. 재정보조를 위한 신청서는 최대한 이른 시기에 제출해야 하며, 최초의 서비스 후 청구내역서 일자로부터 최대 240일 기간까지에 대하여 접수받습니다. 완전한 재정보조 신청서가 최초 내역서 보다 240일 이전에 제출되면, Inova는 FAP-자격에 대한 결정을 내리고 당사자의 어느 부분에 대한 보조 자격이 있는지에 관하여 당사자에게 서면으로 통지합니다. 완전히 작성되지 않은 재정보조 신청서가 접수되면, Inova는 신청서를 완성하기 위해 필요한 정보 또는 문서를 적시하는 서면으로 된 통지서를 발부합니다.

재정보조 결정은 최초의 적격 서비스가 시행된 날로부터 6번째 달의 말일까지 유효합니다. 환자가 이 6개월 기간 도중에 재정보조를 다시 신청할 필요는 없습니다; 그러나 환자가 입원환자 서비스로 전환하기 위해서는 당사자에게 자격이 있을 것으로 보이는 정부 지원 또는 정부 보조 건강보험을 신청할 것인지에 대한 질문을 받을 수 있습니다. 재정보조 신청서의 정리된 파일이 유효하지 않게 될 경우 유자격 서비스의 각 종료 시기에 재정보조 필요성은 다시 평가합니다. Inova의 인간 존엄성과 봉사정신의 가치는 신청 절차와 재정 필요 결정, 재정보조의 지급 과정에서 반영되어야 합니다. 재정보조 요청은 즉시 진행해야 하며 Inova는 환자 또는 신청자의 자격 결정에 관하여 작성된 신청서가 접수된 날로부터 30일 이내에 서면으로 통지할 수 있도록 합당한 노력을 기울여야 합니다. Inova의 재정보조를 받기 위하여, 당사자는 요청받은 정보를 시의적절하게 제공하며, 정부 지원 또는 정부 보조 건강보험 도는 자신에게 자격이 있는 것으로 보이는 이 밖의 여하한 보험 프로그램에 신청하고, 해당 환자와 Inova가 설정한 지불 계획에 의해 의한 모든 금액을 지불하여 협조하여야 합니다. 재정지원을 추구하는 사람에게만 해당하는 자료의 정보 사용 자격을 결정할 때, Inova는 재정지원을 추구하는 사람의 자료에서 얻은 정보를 사용할 수 있습니다. 여기에는 시스템에서 얻은 다음과 같은 데이터가 포함될 수 있습니다:

• Inova를 도와 재정보조가 필요할 수 있는 사람을 찾고, 따라서 보조 신청서를 받을 수 있도록 돕는 것;

• Inova가 재정보조 신청서를 제출한 사람들의 정보 정확성을 확증하도록 돕는 것; • Inova가 기왕에 보험가입이 되지 않은 특정 환자가 이미 메디케어드 또는 기타 제삼자 포괄 대상이 되는지 확인하도록 돕는 것; 그리고/또는

• Inova가 재정지원 적격성 결정 취지로 접수할 수 있는 정도의 소득 또는 기타 문서가 기왕에 있는 사람은 안전망 조직(연방이 인정하는 보건소) 환자로서 확인합니다.

미불 발생 시 절차 이 정책에 의한 재정지원을 신청하지 않는 환자는 Inova의 일반적인 비용청구와 징수 절차 대상이 됩니다. Inova의 회계에서 모든 환자 책임 부분은 Inova의 청구와 징수 체계에 따라 처리되며, 이는 연방 지침 범위 이내에서 징수할 때 효력이 있습니다. 환자 책임 부분은 사전 청구, 내역서, 그리고 후속 조치로 이뤄진 절차에 따르며 자동적이고 체계적인 단계에 의해 처리됩니다. 모든 징수 행위는 정당한 부채징수법과 ACA의 윤리와 전문적 책임에 관한 국제 강령을 준수하여 이뤄집니다. 환자는 120일에 대한 내역서를 받을 것입니다. 120일 이후에 해당 계정은 환자와의 접촉과 수용 가능한 지불 기간의 설정 없이 전문적인 징수 기관으로 이전될 것입니다. 미지불 상태가 합리적인 기간을 초과하여 연장되면 Inova는 해당 계좌에 대한 추가 조치를 취할 수 있습니다.

환자가 지불 합의서를 작성하고자 하는데 전액 지불 가능한 자원이 없다면, 이 환자는 Inova가 이 상황을 알 수 있게 해야 하며, Inova는 지불 계획을 제안할 것입니다. Inova의 기관이나 제삼자 징수 기관 가운데 어느 곳도 환자가 이 정책에 의한 재정지원 대상 자격이 있는지 결정하기 위한 합당한 노력을 먼저 기울이기 이전에 법적 행동 또는 적대적인 신용 보고와 같은 특별한 징수 조치 (Extraordinary Collections Actions ("ECA"))를 취하지 않습니다. 이 합당한 조치에 포함되는 것은:

• 최초 청구 내역서 발부일로부터 적어도 120일 동안은 어떠한 ECA 조치도 취하지 않을 것임을 확인 함.

• Inova의 재정지원 정책에 관한 안내와, 미지불 상태가 발생하면 모든 의도적인 ECA 조치가 취해질 것이라는 서면으로 된 30일 기간 통지를 제공함.

• 미지불 상태가 발생하면 모든 의도적인 ECA 조치가 취해질 것이라는 통지를 구두로 제공함.

• 지불금이 접수된 이후 또는 ECA 조치가 취해진 이후에 환자가 재정지원 대상 자격이 있다고 결정될 경우에는 해당 계정이 조정되고 징수 조치는 번복됩니다.

• 수익주기운영본부 부본부장 또는 그의 지정인은 Inova가 어떤 사람이 FAP-유자격자인지 여부와 한 개 이상의 ECA 조치가 취해져야 하는지 여부에 대하여 합당한 조치를 취했는지 판단할 책임이 있습니다.

VI. 정책 관련 안내 Inova가 이 정책을 널리 홍보하는 방법 Inova는 각 Inova 기관들과 함께 본 재정보조 정책을 널리 홍보할 것이며, 여기에는 다음 방법이 포함됩니다.

• Inova는 전체 시설에서 본 재정보조 정책 (Financial Assistance Policy (FAP))에 대한 링크를 마련할 것이며, 재정보조 신청서와 일반 언어로 된FAP 요약 설명서를 각 병원 시설과 포괄 기관에서 사용할 수 있도록 웹사이트를 만들 것입니다;

• 본 정책, 재정보조 신청서, 그리고/또는 일반 언어 요약설명서는 요청에 대한 경비 부담 없이 공공 장소에서, 그리고 우편으로 구할 수 있도록 할 것입니다;

• FAP의 일반 언어 요약본은 입원 또는 퇴원 절차의 일부분으로써 제공하여 보험 미가입자, 보험 불충분 가입자, 또는 자가 지불로 분류된 사람들이 이 정책 안내를 받을 수 있도록 할 것입니다;

• FAP, 재정보조 신청서, FAP 일반언어 요약본은 중요 인구(각 Inova 병원 시설 구역이 담당하는 1,000 명 미만 또는 5퍼센트 미만의 사람들)를 위해 번역할 것입니다;

VII. 기타 규정 정보

일반적인 청구금액 (Amounts Generally Billed (AGB)) Inova Health System은 재정보조에 적합한 어떤 사람에게도 일반적인 보험 가입자 청구 금액보다 많이 청구하지 않을 것임을 확인합니다. Inova Health System은 이 재정지원 정책의 최저 할인율을 제시할 때 코드 501(r)조 규정에서 설명하는 룩백 방식을 사용합니다. 이 방식에 의하면 FAP-유자격 당사자에 대한 최저 할인율은 각 Inova Health System 병원 시설과 포괄 대상 기관에서 다음과 같이 제공해야 합니다:
