Patient de 51 ans

Adénocarcinome du poumon droit T3N0M0 pneumonectomie droite en août 2008 chimiothérapie

Janvier avril 2009 : 4 épisodes infectieux respiratoires traités par antibiothérapie en ambulatoire

5ème épisode en mai : épisode plus sévère avec dyspnée motivant son hospitalisation

A J1, épisode de dyspnée au lever

suspicion d’embolie pulmonaire

angioTDM pulmonaire : négatif

Déviation médiastinale Foyer de pneumopathie gauche

Etirement veine pulmonaire Etirement bronche lobaire inférieure

inférieure gauche

Syndrome post-pneumonectomie

Quelques jours plus tard, nouvel épisode de dyspnée au lever

Suspicion syndrome orthodéoxie-platypnée

Echographie trans-oesophagienne : FOP avec shunt droit-gauche permanent

Repositionnement médiastinal par prothèse siliconée

Suites opératoires simples

autopsiques)
