MOYENS DE PREVENTION D’UNE IRA AUX PODUITS DE
CONTRASTE
I/ EPIDEMIOLOGIE :
Persson PB et al. Contrast medium induced nephropathy; the pathophysiology Kid. Int. Suppl. 2006
avec durée d’exposition
Δ fonction osmolarité du PCI
Activation systeme RTGMédiés par: adénosine, endotheline, NO, Pg..
IV/ MOYENS DE PREVENTION:1- Identification des Facteurs de risque:

Incidence 14.5 à 55%Mehran et al. Contrast induced nephropathy : definition epidemiology and patient at risk. Kidney Int. Suppl. 2006
Incidence : 5.7 à 29.4%Nikolsky E et al. Impact of chronic kidney disease on prognosis of patients with diabetes mellitus treated with percutaneous coronary intervention. Am. J. Cardiol. 2004

Type de PCI injecté: hyperosmolaires: 2000mosmol/kg
PCI faible osmolarité: 600 à 800 mosmol/kg iso: 290 à 300 mosmol/kg
Incidence plus faible avec PCI faible et iso-osmolaire-Aspelin P, et al.Nephrotoxicity in high-risk patients study of iso-osmolar and low osmolar non-ionic contrast media study

effects in high-risk patients undergoing angiography. N Engl J Med 2003

Recommandations Am. College of Cardiology et Am. Heart Association: recourir PCI iso-
Pathogenesis and prognostic implications Arch. Intern. Med. 1990

validation. J. Am. Coll. Cardiol. 2004
IV/ MOYENS DE PREVENTION:2- Mesures préventives globales
Arrêt médicaments néphrotoxiques (AINS++)
IV/ MOYENS DE PREVENTION:3- Expansion volémique++ :
Seule mesure préventive formellement établie !
Quel soluté? SSI 0.9% . Mueller C. Prevention of contrast induced nephropathy with volume supplementation. Kidney Int. Suppl. 2006
. Taylor AJ, et al: Preparation for Angiography in Renal Dysfunction: a randomized trial of inpatient vs outpatient hydration protocols for cardiac catheterization in mildto-moderate renal dysfunction. Chest 1998
Bicarbonate de sodium: Merten GJ et al. Prevention of contrast induced nephropathy with sodium bicarbonate : a randomized controlled trial. JAMA 2004Ho KM et al. Use of isotonic sodium bicarbonate to prevent radiocontrast nephropathy in patients with mild pre existing renal impairment: a meta-
analysis Anaesth. Intensive Care 2008
Volume d’administration optimal??- SSI: 1ml/kh/h
A adapter fonction cardiaque (bilan hydrosodé équilibré)
pharmacologiques: Table 2
Pharmacologic agents evaluated for contrast-induced nephropathy risk reduction● Positive results (potentially beneficial)—Theophylline/aminophylline—Statins—Ascorbic acid—Prostaglandin E1
● Neutral results (no consistent effect)—N-acetylcysteine—Fenoldopam—Dopamine—Calcium channel blockers (OE Amlodipine,OE Felodipine, OE Nifedipine,OE Nitrendipine)—Atrial natriuretic peptide—L-Arginine
● Negative results (potentially detrimental)—Furosemide Consensus working panel strategies to redice the risk of
—Mannitol contrast inducednephropathy, Am.J. Cardiol. 2006
—Endothelin receptor antagonist
N ACETYLCYSTEINE:
- Controversé++, 27 études !!
