Monsieur L, 78 ans, est amené par le SAMU pour suspicion d’AVC. A
son arrivée, le patient est confus et fébrile à 38,7°C. L’IRM cérébrale
réalisée en urgence est normale mais la bandelette urinaire détecte
la présence de leucocytes et nitrites. L’examen cyto-bactériologique
des urines (ECBU) réalisé au laboratoire retrouve une leucocyturie
significative supérieure à 106 leucocytes/ml et la présence de
quelques bacilles à Gram négatif.
Le diagnostic de syndrome confusionnel sur pyélonéphrite à bacille à
Gram négatif est posé. Le Patient est hospitalisé en unité de soin
intensif et une antibiothérapie probabiliste par Rocéphine est
débutée.
Un exemple
La culture de l’ECBU retrouve 107 Escherichia coli/ml.

2. une diminution du diamètre d’inhibition aux carbapénèmes (parfois uniquement
à l’Ertapénème, carbapénème possédant la meilleure sensibilité pour la détection
des Entérobactéries productrices de carbapénèmase (EPC))
3. la résistance contact à la Témocilline (très bonne valeur prédictive pour ce type
d’enzyme).
4. l’absence d’autres β-lactamases, ces enzymes n’hydrolysent pas les
céphalosporines de 3ème génération (C3G) et faiblement les carbapénèmes.
5. Les souches qui les expriment ont toujours des CMI à la limite de la sensibilité à
ces molécules

- Sous Rocéphine, le syndrome confusionnel et le sepsis ont rapidement
régressé. M. L. a pu retourner à son domicile après 2 jours d’hospitalisation
avec un relai per os par Oflocet pendant 21 jours.
- Aucune notion de voyage ou d’hospitalisation récente n’a pu être mis en
évidence, suggérant une acquisition communautaire de cette souche de E. coli
EPC.
- La présence d’une carbapénèmase sans aucune autre résistance associée n’est
pas une rareté car le plasmide épidémique portant le gène blaOXA-48 ne porte
aucun autre gène de résistance. Ce plasmide est auto-transférable à très haute
fréquence, ce qui peut conduire à l’isolement chez un même patient de
plusieurs espèces d’entérobactéries différentes produisant cette
carbapénèmase.
- Contrairement à Klebsiella pneumoniae, EPC la plus fréquemment isolée et
espèce essentiellement nosocomiale, E. coli possède un risque
potentiellement élevé de diffusion communautaire, ce d’autant que les
phénotypes ne sont pas toujours évidents à mettre en évidence.
