Notre travail r6sume l'exp6rience du service de Gastroent6rologie et d'Endoscopie digestive de l'h6pital de San Jos6 de Bogota en ce qui concerne les ict~res et particuli~rement quant ~ l'usage de la laparoscopie et de I'E.R.C.P. dans cette pathologie.

Presenting the experience o] G.L unit of San lose Hospital from Bogota, we conclude that E.R.C.P. either Okuda's method or peritoneoscopy complement each other.
1. I N T R O D U C T I O N
L'6tude du syndrome ict6rique est caract6- ris6 non seulement par ses difficult6s intrin- s6ques, mais aussi par les suites qu'une attitude inad6quate peut avoir, en faussant le dia- gnostic. L'essentiel est de d&erminer le carac- t~re m6dical ou chirurgical de l'ict~re et d'en ddduire le type de traitement ~ envisager. C'est dire l ' importance du diagnostic 6tiologique.
II. M A T E R I E L E T M E T H O D E S
L'analyse a port6 sur 322 cas de malades consultant pour ict6re, vus entre le I er janvier 1970 et le 1"" d6cembre 1979, au service de
Gastroent&ologie et d 'Endoscopie de l'h6pital San Jos6. De ce groupe, sont 61imin6s 44 cas, par manque de pr6cisions. Cette 6tude ne porte donc que sur 278 cas. Nous avons distingu6 deux groupes :
1) Les malades vus du 1-1-70 au 31-12-77 ;
2) Les malades vus du 1-1-78 au 1-12-79.
Les chiffres de bilirubine totale ont oscill6 entre 1,5 mg % et 32,9 mg %. La dur6e d'6volution des syndromes, assez longue, a 6galement vari6 allant en g6n6ral de 3 jours minimum ~ 10 ans maximum. I1 faut compren- dre bien sOr que dans cette derni6re 6ventua- lit6, il s'agissait d'ict6res intermittents. Les m6- thodes utilis6es ont 6t6 essentiellement la laparoscopie et I 'E.R.C.P. tandis que la cho-

langiographie transcutan6e (Okuda), l 'ultra- sonographie et la tomographie axiale n 'ont 4t6 utilis6es qu 'en compl6ment.
que le pourcentage d 'exacti tude s'est ensuite 41ev6 h 89 %, grfice h l 'exp6rience acquise par le personnel du service et la modernisat ion de nos appareils de radio.
I I I . R E S U L T A TS
S E L O N L E S T E C H N I Q U E S
On peut observer (tableau I) que chaque technique d6termine un tableau de fr6quences 6pid6miologiques sp6cifiques. II est 6vident que la laparoscopie permet d 'appr6cier une plus grande fr6quence de diagnostics positifs dans les cas concernant la pathologie primaire du parenchyme h6patique, par exemple les h6patites, oh l 'on peut distinguer diff6rents types et degr6s d 'dvolution ; les cirrhoses, les tumeurs primaires et secondaires, les proces- sus d 'obstruct ion biliaire qui par leur 6volution, peuvent alterner l 'aspect macroscopique de la glande. Dans ces cas, la diff6renciation entre chol6stase intra-h6patique et extra-h6patique d6termine la conduite ~ tenir : intervention ou abstention, m6me en l 'absence de diagnostic de la maladie primaire. C'est dans les cas o6 l 'ob- servation clinique et paraclinique fait soupqon- ner une cholestase extra-h6patique que l 'opa- cification des voles biliaires par vole r6trograde et transcutan6e acquiert sa valeur maximale, permettant de d6celer oddites, tumeurs, lithia- ses, fistules ou st6noses.
Dans d 'autres cas, ce proc~d6 d 'opacification permet d '6carter les d6fauts de drainage des voies biliaires, orientant ainsi l '6tude vers une autre pathologie, sp6cialement h6pato-cellu- laire. Soulignons que si la laparoscopie permet le diagnostic d 'une chol4stase extra-h6patique et d6termine ainsi une conduite chirurgicale, m6me sans diagnostic exact, la cholangiogra- phie montrant l'int6grit6 des canaux, oriente vers une maladie h traitement m6dical.
Nous observons au tableau II <~ Laparo- scopie >> que sur un total de 89 examens, 75 (soit 84,2 % ) ont abouti h un diagnostic cor- rect ou du moins ~ une orientation juste (an- n6es 1970 ~a 1977) et que ces deux derni6res ann6es, le pourcentage s'est ~lev6 ~ 88 %.
Jusqu 'en 1977, 66 malades furent 6tudi6s par E.R.C.P. (tableau I I I ) et sur ce nombre, on a conclu un diagnostic positif dans 42 cas (63,3 % ) mais on voit, dans ce m~me tableau,

Le tableau IV rdv61e un indice de positivit6 de 89 % avec l 'emploi des deux mdthodes. On remarque ici encore l'616vation du pour- centage d'exactitude de 79 % ~ 89 % du pre- mier au deuxi6me groupe. Nous en concluons que les deux mdthodes ne s'excluent pas mais se compl6tent.

I . I N T R O D U C T I O N
The approach to jaundice has been cha- racterized not only by the intrinsic difficulty in itsell in certain cases, but because ol the consequences that the improper management oj these patients may have if a wrong diagnosis is made.
It is really important to determine if jaun- dice will have medical or surgical treatment, and in each ol these cases what is" the most convenient treatment, which shows the im- portance oJ etiological diagnosis.
I I . M E T H O D S A N D M A T E R I A L S
322 cases were analyzed in a period bet- ween the 1st o[ January 1970 and 1st o[ De- cember 1979, with jaundice as main problem, in the Gastroenterological and Endoscopic Unit at the San Jose Hospital Jrom Bogota, which are separated in two main Groups :
1. Until December 1977 ;
2. From that date to the 1st oJ Dec. 1979.
44 were not available to study because of unsuJJicient clinical data : N = 278.
Total bilirubins were between 1.5 mg % and 32.9 mg % evolution was between 3 days and 10 years. Obviously, in this last event the episodes were recurrent.
Ages were between 14 ans 84 years.
We essentially used as diagnostic methods : the peritoneoscopy, E.R.C.P. ; T.P.C. with the thin needle (Okuda's); ultrasonography and axial tomography were only used as comple- menta;~y methods.

T O T H E M E T H O D S
In table I, it is easy to recognize that each method has a certain specificity according to the pathology. It is evident that with perito- neoscopy, the high frequency corresponds to primary hepatic pathology, like in the hepatitis, and in this speciJic case, we are able to de- termine the type and the degree o] evolution
o] the hepatitis : cirrhosis, primary and meta- stasic tumors and obstructive syndromes ; in these cases, di]ferentiation ol intra or extra- hepatic cholestasis will give the approach to medical or surgical treatment, even without etiological diagnosis.

When clinical and paraclinical data suggest extra-hepatic cholestasis, it is very important to opacify the biliary tree by means of E.R.C.P. or Okuda's method. We found under these cir- cumstances, from odditis to intraparenchyma- tous tumors (see table I).
In other cases it was useful to demonstrate the permeability of the biliary tree orientating the study specially to hepatocellular pathology.
The table I I shows, that out of 89 perito- neoscopies we had a positive diagnosis or at least a very sure guide in 75 patients (84.2 %) and 14 patients (15.8 %) with wrong diagnosis, until 1977, but with more experience, our average o~ correct diagnosis rised up to 88 % as we see in the same table.

We believe that a right diagnosis is given, even when, without etiological diagnosis, the data is sufficient to suggest surgical treatment.
Peritoneoscopy was useful to avoid laparo- tomies like in peritoneal carcinomatosis or cholestatic hepatitis.

With E.R.C.P. (table HI) we studied 148 pa- tients, and in 142 (63.6 %) we had a right diagnosis, in 8 ( 1 2 . 1 % ) diagnosis was wrong or inclompIete, and in 16 (24.2 %) E.R.C.P. was not performed because of anatomic cha- racteristics of duodenum or Vater's ampulla or because we canalized pancreatic duct when we looked for choledocus, that until 1977, but we see that with a more skilled staff, the posi- tivity of diagnosis rises to 89 %.
In table I V we describe 89 % of positivity with peritoneoscopy and E.R.C.P. Also, we note that the positivity rises from 79 % in 1977 to 89 % in 1979, at the same time we are able to say that both methods are not excluent, on the contrary they complement each other.
