« As geriatricians, our concern is the frail
old patient »

Acute disease and hospitalization
increase risk of FD
1/3 of patients aged 75 y and older lost their abilities in ADL between their pre-admission level and discharge 1,2
3 months after discharge, 1/3 of patients declined in ADL
compared to their pre-admission level.3

60 % of FD 4

Why identifying older people at risk in the emergency departement or as soon as possible ?
Targeting
According to the limited number of geriatric beds
« The right patient in the right bed »
Because geriatric interventions are useful for frail patients (not too
independent, not too dependant)
Avoiding waste of time
Hospital-associated functional decline begins within 48h after
admission

En pratique :
Two- step approach
1. Screening : Outil rapide, bonne sensibilité
ISAR, SHERPA, TRST patient âgé aux urgences
EFS patients âgés avant chirurgie
G8, VES-13 patients âgés en oncologie
?? En médecine générale : EFS, Groningen FI

A l’hôpital :

Take Home Message
