• 10 patients à l’inclusion avaient une fibrose avancée/cirrhose (Ishak fibrosis score=4, 5 or 6)
– Tous les patients ont eu une amélioration du score Ishak fibrosis (amélioration ≥1-point )
– 4 patients avaient une cirrhose à la PBH , tous ont amélioré le score d’Ishak de fibrose

Introduction
• Le taux de disparition spontanée de l’Ag HBs est estimée
0.5% à 1.7% par an
• Ce taux varie en fonction des populations
• Au cours de la phase III de l’ étude ETV-022, 5.1% des
patients Ag Hbe + traités par ETV ont eu une disparition
de l’Ag Hbs à S 120
• Dans ce travail il a été evalué la perte de l’Ag Hbs en
fonction des génotypes du VHB

Méthodes
• L’ Ag HBs a été recherché régulièrement durant le traitement et le suivi par la méthode Abbott AxSYM microparticle enzyme immunoassay
• La perte de l’Ag Hbs est affirmée sur 2 mesures consécutives durant le traitement .

Combinaison entecavir + tenofovir
• Suivi multicentrique ouvert de 23 malades avec antigène HBe positif et 16 malades avec
antigène HBe négatif multirésistants ou répondeurs partiels à des traitements précédents
par analogues nucléo(t)idiques et présentant une maladie hépatique avancée, traités par
la combinaison ténofovir (245 mg/j) et entécavir (1 mg/j)

Conclusions
• Efficacité virologique : Réduction de plus de 90% de la
Charge virale .
• Données de résistance à 6 ans = 1%
• Amélioration histologique à long terme
Amélioration des lésions nécrotico-inflammatoires et du
score de fibrose
• Chez les répondeurs partiels : Suppression de la charge
virale : Association ETV - TDF
• 5% des patients traités par ETV présente une perte de
l’Ag Hbs , patients Génotype A –D , 9.8% (13/132) ETV
/ 6% (9/150) LAM
