• CONCLUDING RECOMMENDATIONS • In patients with mild-to-moderate levels of acute pain, the recommendation is to
initiate treatment with a maximum dose of 4 g/day paracetamol, with a minimum 4 h interval between each 1 g dose (step 1).
• Patients who do not achieve sufficient analgesia within 1 – 2 days should switch to a selective COX-2 inhibitor or, if gastrointestinal safety and risk of bleeding are not an issue, a NSAID (step 2).
• If analgesia continues to be inadequate, the recommendation is to add tramadol, or combination therapy with acetaminophen plus either codeine or tramadol (step 3).
• Patients reporting severe pain should be referred to a pain clinic or specialist for treatment with an opioid analgesic. (step 4).
• Treatment with an oral selective COX-2 inhibitor or topical ns-NSAID gel or cream, or a combination of oral and topical treatments was recommended for patients with acute pain from musculoskeletal injury due to sport or other trauma.
• They also stressed the importance of early and aggressive treatment of acute pain in order to reduce the risk of pain becoming chronic or neuropathic in nature.
• Nonsteroidal anti-inflammatory drugs • Nonsteroidal anti-inflammatory drugs are recommended as second-line analgesics in the Dutch guidelines [1,2], and as first-
line treatment in the ACP/APS guidelines [3,4]. Previous systematic reviews have found no evidence to support the use of NSAIDs in the treatment of sciatica [2,15–17], but in a more recent Cochrane review, NSAIDs were slightly but significantly more effective than placebo in producing short-term (3 weeks) reductions in pain intensity (mean treatment difference –8.39, 95% confidence interval [CI] –12.68, –4.10) and the proportion of patients with acute low back pain experiencing global improvement (risk ratio [RR] 1.19, 95% CI 1.07, 1.33). They were also significantly more effective than placebo in reducing chronic low back pain (mean treatment difference –12.40, 95% CI –15.53, –9.26) [18]. There were no significant differences in efficacy, in terms of pain reductions or global improvements, between NSAIDs and paracetamol, or between NSAIDs and other drugs, including narcotic analgesics and muscle relaxants [18]. Treatment with NSAIDs was associated with a significantly higher incidence of adverse events, compared with placebo (acute back pain: RR 1.35, 95% CI 1.09, 1.68; chronic back pain: RR 1.24, 95% CI 1.07, 1.43) or paracetamol (RR 1.76, 95% CI 1.12, 2.76). Cyclooxygenase-2 (COX-2)-specific inhibitors were comparable in analgesic efficacy to nonselective NSAIDs in patients with acute low back pain (mean treatment difference -1.17, 95% CI -4.67, 2.33), but were associated with a lower risk of adverse events (RR 0.83, 95% CI 0.70, 0.99) [18]. There was no evidence that any nonselective NSAID was superior in efficacy to the others.
• Nonsteroidal anti-inflammatory drugs are associated with a number of well-documented adverse events (Table 8.1), including gastrointestinal disturbances such as dyspepsia or nausea, and alterations in renal function. Such adverse events are usually mild or moderate in severity [18], although serious gastrointestinal complications such as bleeding or ulceration can occur [19]. Selective COX-2 inhibitors are associated with a lower risk of gastrointestinal adverse events than nonselective agents [20]; however, some of these agents are associated with an increased risk of cardiovascular adverse events [21,22], although it is unclear whether this is a significant concern during the short treatment periods recommended for sciatica [18].

Dans les 24 heures suivant la chirurgie, la consommation totale d’analgésiques opioïdes a été significativement réduite dans le
groupe traité par Celebrex comparativement au groupe placebo (p = 0,009). Celebrex a été associé à des réductions significatives
de la consommation d’analgésiques opioïdes comparativement au groupe placebo de 10 à 12 heures (p = 0,005) et de 12 à 24
heures après la chirurgie (p = 0,012). Le pourcentage de patients du groupe placebo (41 %) qui ont requis des analgésiques
opioïdes a été significativement plus élevé que le pourcentage de patients traités par Celebrex qui ont requis des analgésiques
opioïdes (22 %; p = 0,008) de 10 à 12 heures après la chirurgie. Davantage de patients du groupe placebo (37 %) que de patients
traités par Celebrex (18 %) ont présenté des événements indésirables. Les incidences d’événements reliés aux analgésiques
opioïdes, tels que des troubles du système nerveux central (SNC) (12 % c. 3 %, respectivement) et la constipation (5 % c. 1 %,
respectivement), ont été plus élevées chez les patients du groupe placebo que chez les patients traités par Celebrex.

Dans les 24 heures suivant la chirurgie, la consommation totale d’analgésiques
opioïdes a été significativement réduite dans le groupe traité par Celebrex
comparativement au groupe placebo (p = 0,009).

Conclusions • Effets gastrointestinaux (GI) indésirables des anti-inflammatoires
non-stéroïdiens (AINS)
– Hémorragie et perforation du TGI supérieur et décès1,2
– De 10 % à 60 % des patients peuvent présenter nausées, dyspepsie, anorexie, douleurs abdominales, flatulences et diarrhée3
• Les patients ne sont pas tous exposés au même risque.
– Antécédents d’ulcère hémorragique, prise de plusieurs AINS (aspirine), doses plus élevées d’AINS, anticoagulants, antécédents d’ulcère non compliqué, âge > 70 ans, H. pylori, stéroïdes4-6
– Le risque apparaît dès la 1re dose. 7,8
• Les AINS ne comportent pas tous le même risque.9
• Aux patients exposés au risque, on devrait prescrire un inhibiteur sélectif de la COX-2 en monothérapie ou une association d’un AINS classique et d’un inhibiteur de la pompe à protons (IPP), alors qu’aux patients exposés à un risque élevé, on devrait prescrire une association d’un coxib et d’un IPP.10

• Pregabalin • Pregabalin is also an analogue of GABA and is a structural analogue of gabapentin [43,44]. It acts in the same way as gabapentin [45] and has been shown to inhibit the
release of various neurotransmitters involved in pain pathways, including glutamic acid, noradrenaline, serotonin, dopamine and substance P [43]. Pregabalin has the benefit of linear pharmacokinetics [46], allowing for easier dose control [45]. The dosing for pregabalin is twice daily and it has a 6-fold higher affinity for the α2-δ subunit of the voltage-gated calcium channel than gabapentin [45]. In clinical trials, pregabalin has been shown to be effective in the treatment of neuropathic pain, including diabetic neuropathy, postherpetic neuralgia, and central pain, and is recommended as first-line treatment for these conditions [38].
• Several clinical trials have investigated the efficacy of pregabalin in the treatment of lumbar or cervical radicular pain. In one study, conducted in a primary care setting, 1304 patients with cervical or lumbosacral radiculopathy were treated with pregabalin, alone or in combination with other analgesics, or alternative analgesics for 12 weeks; all patients had experienced chronic neuropathic pain for at least 6 months, which was refractory to previous analgesic therapy [47]. The study was conducted under real-world conditions, meaning that the treatment administered was decided by the physician on the basis of the patient’s condition rather than by randomisation *47+. The mean daily pregabalin doses were 187 mg in the monotherapy group and 191 mg in the combination therapy group. Compared with patients who did not receive pregabalin, both the monotherapy and combination therapy groups showed significantly greater reductions from baseline in pain and improvements in health-related quality of life (Figures 8.2 and 8.3) [47]. The mean pain reductions were 56% in patients receiving pregabalin monotherapy and 51% in those receiving combination therapy, compared with 36% in patients who did not receive pregabalin (P
