1) Telephone 9-1-1 immediately. 2) Once 9-1-1 has been called, a doctor will be sent to the home in order to declare the death and complete the Quebec Declaration of Death (SP-3) document. Alternately, the declaration of death is sometimes performed over the telephone between the doctor and the ambulance technician on site. 3) In order to proceed with donation, the doctor/technician will be required to complete and sign the Criteria Form (Fiche critères) for McGill University (attached), with the family's assistance. 4) If all the criteria for eligibility are met*, the family or doctor can call the McGill Body Donor Program directly at 514-398-2575 to request a transport. If the declaration of death occurs outside of our office hours (Monday-Friday, 9am – 5pm), a call should be placed directly to Blythe- Bernier Funeral Home at 514-495-8082, available 24/7 for transport to the University. In cases of death at home, transport to the university must take place within a maximum of 6-8 hours following death. 5) The body donor card (signed), the completed Criteria Form (Fiche critères), and the Declaration of Death (SP-3) document should be sent with the body for transport to the University. As above, if the declaration of death occurs outside of our office hours (Monday-Friday, 9am – 5pm), a call should be placed directly to Blythe-Bernier Funeral Home (514-495-8082), our affiliated funeral home, available 24/7 for transport to the University. As in cases of death at home, it should be noted that death in a medical residence other than a hospital is subject to the same time limit of a maximum of 8-10 hours for transport to the University, owing to the absence of refrigeration facilities in the majority of non-hospital residences. 4) The body donor card (signed), the completed Fiche critères, and the Declaration of Death (SP-3) document completed by a doctor should be sent with the body for transport to the University. *In addition to the criteria indicted on the attached Criteria Form (Fiches critères), the University reserves the right to limit the number of body donations accepted during periods of exceptionally high demand, without notice beforehand.
