• Suivi > 10 ans ATCD: insuff. Cardiaque
• Autonome s’occupe de son mari, soeur, petits enfants
• ++détérioration dernière année
• Bili 26, albumin 24, INR 1.63. Cr 120;
• Ascite réfractaire (intractable)

• 1 an plus tard: Bili 22, albumin 39, INR 1,26
• Aucun ascite
• Surveillance CHC
#95 improving liver function and delisting of pts awaiting LT • France Tx network
• 106 HCCs (low natural MELD) + 77 decompensated cirrhosis
• 78% CP B/C Avg CP 7.5 & MELD 20
• SVR 84%
• 6% decomp during therapy
• 1/3 no change; 1/3 partial improvement; 1/3 complete response (EH, ascites, bili) after avg F/U 24 wks
• 16% delisted (few if MELD > 20 - see #1077)
Conclusions
• Succès possible (même probable) • Résultat RVS plus faible • Décision de traiter: risques/bénéfices en fonction des co-
morbidités/MELD…greffe ou pas greffe? MELD PURGATORY • Consentement bien éclaire (+/- famille) • Également traiter les manifestations de la cirrhose decompensée • Suivi serré (fréquent) durant le traitement:
• Acute on Chronic Liver Failure (ACLF) • ? Drug Induced Liver Injury (DILI) • Toxicité médicamenteuse
