- amélioration du pronostic par une prise en charge
thérapeutique plus précoce

- Grande sensibilité pas de fausse réassurance
- Grande spécificité pas d’anxiété inutile
- Morbidité liée au test minimale
- Acceptable par « patient » (asymptomatique) et par médecin

- Réduit la mortalité liée au cancer du poumon et/ou améliore la
qualité de vie

Risque potentiel du dépistage du cancer

- Utilisation de techniques parfois invasives (endoscopie,

- Anxiété parfois inutile (faux positif)
- « Surdiagnostic » = détection d’un cancer pulmonaire qui s’il
n’avait pas été détecté n’aurait pas affecté la morbi/mortalité du
patient

Conclusions
- RX thorax et CT Scanner permettent la détection de lésions à des stades précoces (CT plus sensible que RX)
mais RX thorax et CT Scanner ont un taux élevé de faux positifs anxiété, procédures inutiles, morbidité
- Etudes randomisées de suivis à long terme ne démontrent pas d’effet du dépistage par RX thorax sur la mortalité
- NLST a démontré aux USA l’intérêt d’un dépistage par CT Scanner 1x/an chez les sujets tabagiques (30 paq/an) en terme de mortalité par cancer du poumon.
- Le résultats des études européennes est attendu
Recommandations
• Les Sociétés savantes américaines (ACCP, ACS, ATS) recommandent le dépistage par CT thorax selon les modalités du NLST
• Les Sociétés savantes européennes réservent leurs recommandations aux résultats des études européennes en cours. Pas de dépistage systématique en dehors des études cliniques

The role of bronchoscopy in the management
of SPN remains limited
because
yield of bronchoscopy in this indication is poor
Limitations of Current Approach to Diagnosis
Standard Bronchoscopy Limitations:
• Sensitivity is 20% - 80% (size, peripheral or
poximal location…)
• Physician must correlate pre-operative CT
with bronchoscopic view
• Generally cannot reach effectively beyond
~5th airway generation
• Essentially a blind procedure
• Use of fluoroscopy – generally lesions
larger than 1cm are detectable

mm de diamètre

But de l’étude :
Comparer le rendement des cryobiopsies et des biopsies transbronchiques à la pince dans le diagnostic des nodules pulmonaires présentant un diamètre entre 8 et 20 mm
METHODES :
20 patients
Endoscopie sous AG guidée par :
- fluoroscopie
- navigation bronchique électromagnétique
- échographie endobronchique
Critères d’inclusion :
• Âge > 18 ans
• Nodule pulmonaire périphérique de 8 à 20 mm de diamètre
• Pet + ou Pet – avec FR
Nodule pulmonaire solitaire
< 8 mm > 20 mm8 - 20 mm
