a. This event must be differentiated from the immune reconstitution inflammatory syndrome (IRIS)b. Certain WHO clinical Stage 3 conditions (e.g. pulmonary TB, severe bacterial infections), may not be an
indication of treatment failure, and thus not require consideration of second-line therapy; c. Some stage 4 conditions ( EPTB: simple lymph node TB, uncomplicated TB pleural disease,
esophageal candidiasis, recurrent bacterial pneumonia) may not be an indicator of treatment failure and thus not require consideration of second-line therapy;
d. Without concomitant infection to cause transient CD4 cell decrease.e. Some experts consider that patients with persistent CD4 cell count <50/mm3 after 12 months on ART
may be more appropriate.f. The optimal viral load value at which ART should be switched has not been defined. However, values of
more than 10,000 copies/ml have been associated with subsequent clinical progression and appreciable CD4 cell count decline.

Objectifs de l’étude
• Objectif principal– Comparer le gain en CD4 chez des patients recevant un
traitement antirétroviral selon l’approche de santé publique et chez ceux traités avec l’approche de référence, dans des hôpitaux de district
• Objectifs secondaires– Comparer entre les 2 approches
• L’efficacité virologique et clinique• La tolérance clinique et biologique• L’observance• L’émergence de résistances virales• L’impact sur la vie quotidienne des patients• L’acceptabilité pour les patients et les soignants• Les performances coût-efficacité
Schéma d’étude
• Essai d’intervention, contrôlé, randomisé, multicentrique, sans insu sur l’intervention, de non infériorité
• 9 hôpitaux de district (province du Centre au Cameroun)
• Randomisation des patients en 2 bras parallèles (approche de santé publique ou approche de référence), stratifiée sur les centres
• 2 x 215 patients• Durée du suivi : 24 mois• Traitement antirétroviral disponible dans le
programme national
